Abstract
In the present work, various causes of potential failures in SME are identified and prioritized. The processes involved inmanufacturing a flange used byXYautomobile industry was studied under the expertise of the core team from the plant and data was collected. The collected data rated on a particular scale, used to calculate the risk priority number (RPN) and prioritize the potential failures. Further, the reasons for highly ranked failures identified using Cause and Effect diagrams. The RPN used to prioritize the risk was calculated using the traditional failure mode and effect analysis(FMEA) methodology. Further, to tackle the vagueness and uncertainty associated with it, fuzzy methodology has been suggested. The rpn is a product of severity of the failure, occurrence and detection of the failure.
